This form is for AFFILIATES of Revenue Enhancement Worldwide’s (REW) Revenue Builders Program. All AFFILIATES are contractually required by REW to use an approved website that REW will be responsible for building, hosting and updating. REW has arranged with me to provide this service to you at minimum cost.
Once you have purchased the site building and hosting package, please continue to fill out the form below so we have the necessary information to service your site properly.
Agreement:
~ This agreement is between Terrence Gallagher, freelance designer, and the REW AFFILIATE designated to market REW Programs to the agreed upon market segment.
~ Submission of payment and/or the form below constitutes agreement to terms on this page.
~ By purchasing the Revenue Builders Site and Hosting Package for $1,797 (includes site duplication, branding, first quarter of site hosting and maintenance) the AFFILIATE understands that the site design is a semi-custom sight based on the LMP Revenue Builders example site provided at inception.
~ This agreement is for one year and will be automatically renewed unless cancelled upon with a 30 day written notice. Billed quarterly, payable in advance.
~ Hosting and maintenance fees will be billed quarterly at $99 per month payable in advance, second billing cycle will be prorated accordingly.
~ All necessary information (see form below) must be submitted at time of order.
~ Assistants will be provided to set-up your email service provider to import from the site server.
~ AFFILIATE will be notified of any changes to site information and/or any programs as they are added or deleted.
~ Any additional services beyond the basics (includes site duplication, branding, quarterly billed hosting and maintenance) will be billed at $100/hr.
~ Terrence Gallagher will not be held responsible for any information provided to post on the web site by AFFILIATE or REW.
~ After AFFILIATE approves site, all sales are final.
